Portfolio Package
Each applicant to the DNP program is required to complete a portfolio package, to include:
1. A completed School of Nursing application.
2. Completed CV with complete work history in Touro University Nevada School of Nursing format. Complete the form by highlighting the first box and typing your name, then tab to the subsequent boxes. Complete only the information that is pertinent to you.
3. A personal goal statement of 300-500 words.
4. A two to three page essay on the role of the advanced practice nurse holding the doctorate of nursing practice. This essay must comply with APA formatting and style guidelines and include citations and references. APA resources are available in the Publication Manual of the American Psychological Association, 5th Edition (ISBN 1-55798-791-2) or at www.apastyle.org.
5. A statement indicating the focus of interest (education, administration, or clinical practice) and potential research interest. Your potential research interest may be broad, but should correlate with your chosen focus of education, administration, or clinical practice.
6. Copies of all graduate-level college transcripts. These must be included in the portfolio and do not have to be official copies. Official copies must be sent to Touro University Nevada per the admissions office requirements.
7. Copies of TOEFL scores must be included if the applicants’ primary language is not English, or if requested by the DNP Coordinator. More information can be found at www.ets.org/TOEFL.
8. A copy of applicants’ driver’s license or state issued identification card.
9. Proof of current registered nurse licensure.
10. Proof of current national certification, if held.
11. A signed copy of the DNP Program Expectations of Students form.
All documents must be arranged in the order listed above and submitted as a single package to the DNP coordinator. Incomplete packages will not be processed.
Additionally, have three professional reference letters completed that attest to your strengths and weaknesses in relation to nursing practice and graduate level education, using the DNP reference form. These should be returned directly to the DNP Coordinator by the person writing the reference.
Once the complete portfolio package and all references have been received an interview will be scheduled with the DNP coordinator and/or other designees. The interview may be conducted by phone if you are a distance student.
If admitted, the following documents must be received by the DNP coordinator no later than one month prior to the beginning of classes:
1. Copy of CPR (AHA HCW) certification
2. Drug Screen
3. Background Check
4. Health Evaluation Form (including all required immunizations)
